THIS CHILD CARE AGREEMENT (the "Agreement") is dated this ________ day of ________________, ________.

IN CONSIDERATION OF the Provider agreeing to provide child care services and other valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the Parties agree as follows:
Details about the child(ren), emergency contacts, and authorized pickups are provided in Exhibit B. Care policies and holiday schedules are provided in Exhibit A.
This Agreement begins on the date specified above and continues until terminated by either Party.

Regular Schedule:
The Parent agrees to pay the Provider USD0.00 per week for child care services.
Payment is due weekly on Friday by check, bank transfer.
The Provider will provide meals and snacks.
The Provider is not authorized to administer medication without specific written authorization.
The Provider represents that it will comply with all applicable laws and regulations governing child care services.
Either Party may terminate this Agreement by providing 14 days' written notice to the other Party.
The Provider may immediately terminate this Agreement if the Parent fails to pay fees when due, or for any conduct that endangers the health or safety of anyone in the Provider's care.
Both Parties agree to keep confidential any personal information about the other Party or the child(ren) that is disclosed during the course of this Agreement.
Any notice required or permitted under this Agreement shall be in writing and delivered by personal delivery, certified mail (return receipt requested), or email.
Notices shall be sent to the Parties at the addresses set forth above, or to such other address as a Party may designate in writing.
No Party may assign this Agreement without the prior written consent of the other Party, and any attempted assignment without consent is void.
This Agreement may be amended only by a written instrument signed by the Parties.
No waiver of any breach of this Agreement will be deemed a waiver of any subsequent breach, and no waiver will be effective unless in writing and signed by the waiving Party.
This Agreement may be executed in counterparts, each of which will be deemed an original, and signatures delivered electronically or by PDF will be binding.
If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions will remain in full force and effect.
This Agreement will be governed by and construed in accordance with the laws of the State of .
The Parties agree to attempt to resolve any dispute arising out of this Agreement through good faith discussions. If unresolved, the Parties will participate in mediation before pursuing litigation.
In any action arising out of this Agreement, the prevailing Party is entitled to recover reasonable attorneys' fees and costs.
This Agreement constitutes the entire agreement between the Parties. Any modifications must be in writing and signed by both Parties.
Sick Child Policy: If a child becomes ill, the Provider will notify the Parent immediately. The Parent is responsible for arranging pickup of a sick child as soon as reasonably possible.
